Membership Program
Louisville Fire Protection District offers district residents a program to help lower the cost of using our ambulance service. The subscription agreement only applies to ambulance services provided by Louisville Fire Protection District.
The advantage to the program is that members will not be charged a copay or deductible after health insurance has completed payment with the district. This membership program is not an insurance policy.

- The ambulance membership program is a simple way to help lower the cost of using an ambulance by prepaying the uninsured portion of the ambulance bill. The advantage of the ambulance membership program is that there won’t be a charge for any co-pay or deductible. The fees collected by your insurance will be considered full payment.
- The membership program is not an insurance policy, only a tool to help with the financial impacts of high co-pays or deductibles.
- Various options are offered to best serve your needs, including individual, family, and senior family memberships, with annual rates starting at $25.
- Membership only applies to ambulance transports from a Louisville Fire Protection District ambulance.
Membership Annual Rates
Membership registration fees are valid for one year. Renewal and registration fees are the same.

Becoming a Member
1. Review The Registration Fee Information Above
Review the categories, descriptions, and annual costs. Fill out the application based on the individuals that will be included in the registration. Please note that all members listed in the application must have insurance.
2. Complete The Membership Application Form
Fill out the application form below and email a copy to membership@louisvillefire.com or mail/drop off a copy at 895 Via Appia Way.
2026 Ambulance Enrollment Form3. Pay Membership Fee
Payment can be made over the phone at 303-666-6595, in person at our station 2 location, or an online invoice via email. Be advised, payment over the phone can only be accepted if an invoice in on file and we process a credit card. To request an invoice, email membership@louisvillefire.com. Fees can be paid via check and mailed to our Station 2 address, 895 Via Appia Way.
4 Membership Renewal will January 1st
Effective starting in 2027, all memberships will require renewal by January 1st. It’s the signer’s responsibility to keep these annual fees current in order to keep the membership from lapsing.
