Permit Applications 

Please complete the plan form below for all permits issued by the Louisville Fire Protection District. Older editions of paper or digital application forms will no longer be accepted. Permit applications for building permits are not required with LFPD if a building permit has been submitted for the corresponding municipality (exception for schools). Additional applications are not required for modifications to existing permits.

Permit Plan Review Application

1. Fill Out The Permit Form

  • Go to the “submit permit application here” tab on this page and complete the form. 

2. Submitting Your Plans

  • The form allows you to upload up to three planning documents with your form.  If you opted to submit your planning files separately, be sure they are emailed to prevention@louisvillefire.com.

3. Pay Your Permit Fees

  • You will be sent an invoice to the contact information provided.

4. Print and Attach to Your Building Plans

  • Once complete your permit will be emailed to you.
